Insurance coverage will be verified prior to you being seen. Co-pays and deductibles will be collected at the time of your visit.
We offer a 30% discount to uninsured patients paying on the date of the visit. Patients in need of a payment plan can contact our billing department at 480-834-0771 prior to their visit.
For your convenience we accept cash, checks, Visa, MasterCard and Discover. There is a $25.00 fee for returned checks.
