Office Hours
All three offices are open Monday – Friday from 8am to 5pm.
Appointments
We have three offices for your convenience. Addresses and phone numbers to our three locations are listed on our home page. Feel free to call any of the numbers to make an appointment. In order to expedite the call make sure that you have your insurance card in front of you when calling us. There is information listed there that we will need.
Once you have made an appointment you will receive a call from our automated system 48 hours prior to your appointment. This system gives you the ability to confirm or cancel your appointment. In the event that you are not able to make your appointment after you have received this call please make sure to call the office where you are scheduled. This courtesy call will allow us to give the appointment to another patient.
First Visit
We will need a copy of your insurance card on your first visit to our practice and/or whenever there is a change to your insurance plan. Please make sure to arrive 30 minutes prior to your scheduled appointment time for insurance verification and completion of paperwork. Copies of intake paperwork are also available by clicking on the specific specialty.
Referrals
Due to the high volume of referrals processed by our staff, as your Primary Care Physician we will require five working days to process a referral to a specialist.
For patients seeing our specialists, it is the patient’s responsibility to obtain a referral/authorization from the Primary Care Physician if their plan requires one prior to seeing the doctor.
Prescription Refills
Please contact your pharmacy first for all refill authorizations. Allow 48 hours for requests to be processed. Except in an emergency case all prescriptions refills will be handled during regular office hours.
